Most driveway problems start under the concrete. Houston's clay swells when it rains and shrinks in a dry summer, so a slab poured on a base that wasn't prepared right heaves, settles, and cracks. We cut to grade, compact the base, reinforce the slab, slope it so water runs off, and cut control joints at the right spacing so any cracking happens at the joints instead of across the middle.
